Special Education Teacher Transition Academy
Summary: A full time Special Education Teacher position is available at Valley Regional High School for the Transition Academy. CT certification 165 (or equivalent) is required. Candidates will have experience working with a wide range of service providers and support staff to meet the needs of diverse learners is preferred. Position requires student case management, close collaboration with families and development of community based opportunities for students to meet transition goals documented within IEPs.
RESPONSIBILITIES: The primary goal of the Transition Teacher is to assist students with achieving their post-secondary goals through assessment of skills, development of a transition plan, identifying job sites and placing students based on strengths and preferences, and supporting families through the linking of resources. Coordinates transition activities for college, career, and community readiness Develops curriculum, designs and implements lesson plans, and assesses student progress. Facilitates referral, application, eligibility process for student and families to adult service providers. Acts as a liaison between the school and community agencies such as Bureau of Rehabilitation Services (BRS) and Department of Disability Services (DDS). Develops and sustains collaborative relationships with potential job sites. Provides direct instruction and consultation to students with disabilities that is designed to meet their individual transition needs. Assists with the development of transition goals and attends IEP meetings. Oversees and trains job coaches who support students placed in job sites Manage ESY (summer program) Manages grants (i.e. Work Force Alliance & Bridge CT) Performs other related duties as assigned Assists in the supervision of transition activities under the direction and supervision of the Building administrator, Director of Pupil Services and Special Education Supervisor.
