A Special Education Case Manager role is available to support students ages 1822 with disabilities in a community-based Transition Program focused on preparing them for life after high school. Located in Elgin, IL, this contract position serves an elementary school setting and emphasizes helping students achieve their individualized Transition Plan outcomes and IEP goals related to employment, education or training, and independent living. Key Responsibilities: Serve as case manager for transition-age students, coordinating services and supports. Develop, implement, and continually monitor IEPs emphasizing transition-related objectives. Design and deliver individualized instruction tailored to each students postsecondary goals. Support development of functional, vocational, and life skills critical for independence. Facilitate community-based learning and job-readiness experiences to build real-world competencies. Collaborate effectively with families, related service providers, job coaches, community partners, and adult service agencies to ensure cohesive support. Program Overview:The Transition Program is uniquely community-centered, providing students with meaningful opportunities such as community-based instruction, job training, and employment experiences. It fosters student-centered planning and hands-on preparation to maximize independence and successful postsecondary outcomes. Qualifications: Valid Illinois Professional Educator License (PEL) with Special Education endorsement (LBS1 required). Experience working with transition-age youth with moderate to severe disabilities preferred. Strong collaborative, compassionate, and patient approach.
