: Oak Street Health Home Visit NPs deliver high-touch, high-quality primary care to our homebound patients. This role is supported by a scribe/medical assistant, nurse, social worker, and other resources. NPs make 6-7 visits daily in a defined territory and collaborate continuously with other team members for care coordination and all other needs. Core Responsibilities: Conduct medical house calls for homebound patients according to defined pathways Optimize and monitor clinical status, identify and address gaps in care, reconciling medications and addressing adherence challenges Collaborate with home health, specialists, social workers, hospitals, SNFs, and specialists Ensure clinical excellence, compliance with regulatory guidelines, and promotion of the Oak Street Health care model Other duties, as assigned
What are we looking for? Genuine passion for primary care and reaching under-resourced communities Strong clinical skills focused on older adults with complex disease states Experience in home-based care (as RN or APN) Excellent communication, collaboration, and problem-solving skills Desire to be a part of a fast-paced, innovative team Have an active, non-probationary state medical license, including US work authorization, if applicable 1+ years APN experience, preferably with a Medicare population Access to reliable transportation Flexibility to travel around the local market Bilingual (English and Spanish) preferred, but not required Working knowledge of Word, Excel and EMRs US work authorization Someone who embodies being 'Oaky'
